Implementation of the 5S Work Attitude Using the Lean Six Sigma Method to the Performance Productivity of Medical Records Unit Officers of Arifin Achmad Hospital, Riau Province

5S (Simplify, Straighten, Scrub, Stabilize, Sustain) is a simple system that can increase employee productivity. The application of the work attitude in storage unit Arifin Achmad Hospital medical record has not been implemented properly. general purpose this study to determine and relationship attitudes using lean six sigma method productivity officer. This uses qualitative approach with other data analysis models according type study. Data used DMAIC (Define, Measure, Analyze, Improve, Control) cycle which was adapted method. performance improvement process based on collected. results problem FMEA (Failure Mode & Effect Analysis) show there were 3 problems / defects frequently occur, namely incorrect stacking an RPN (Risk Priority Number) value 147 (17%), delays returning documents from inpatients ward 112 (30%), distribution errors records another outpatient department 70 (38%). observations in-depth interviews showed overall applied properly officer section. related priority occur section
